1)	The reason I contacted the California Health Care Options office was because I was disappointed with:
		The Customer Service Representative on the phone
		The person helping me at the Health Care Options presentation site
		A mistake made in processing my Medi-Cal needs
		The health plan I had chosen
		The doctor, dentist or medical staff
		The Eligibility Worker or County Medi-Cal staff
2)	After talking with the Health Care Options staff about my complaint:
